Yakima Neighborhood Health Services (YNHS) has been awarded
Recognition by the National Committee for Quality Assurance for Physician Practice Connections® – Patient-Centered Medical Home™ OR PPC®-- PCMH™. YNHS is the first Community Health Center in Washington State to receive this recognition, which acknowledges that YNHS has met the NCQA’s exacting standards in managing patient care based on the medical home model. The medical home model focuses on the partnerships between patients and their families, medical providers, dentists, behavioral health providers and other specialists. The medical provider coordinates and leads the team in caring for their patient- often using the latest technology to share information – which allows patients to get the care they need, in the way they need it.

The PPC-PCMH recognition, valid for three years, is awarded in three levels. In order to receive the level 3 recognition, YNHS met the highest level of criteria in patient communication and access, using electronic technology to organize information and collect data, using evidence-based management for chronic conditions, patient self-management and more.

I could not be prouder of our staff.” YNHS received the Level 3 Recognition for both its 8th Street Clinic and for Neighborhood Connections, the Health Care for the Homeless satellite in Yakima.
